Pay Rate: $2477.03 - $2729.79
1. Review Discharge Waiting Area Criteria
Patient ELIGIBLITY Criteria
- Alert and Oriented x4
- Able to perform ADL's with minimal assist
- 16 years old or greater
- Have discharge instructions and have been discharged out of KPHC
- Patient not a fall risk at time of discharge
- Self Managed oxygen via nasal cannula @ set rate per routine/home utilization
- Patient able to independently administer own medications and have required supplies
- Insulin and non-insulin dependent diabetics who are effectively managing care at home
- Contained body fluids and droplets infection with use of mask
INELIGIBLE patients
" q Active Isolation precautions
" q Aggressive and/or disruptive behavior
" q Patients requiring more than minimal assistance with ADL's
2. Identify Eligible Patients
" · Work closely with charge nurses to identify patients who meet the Discharge Waiting Area criteria.
" · Review all patients with active discharge orders in KPHC and utilize patient records and care plans to assist in the identification process.
3. Communicate with Bedside Primary RN- through Vocera and secure chat
" · Once a patient is identified, promptly communicate with their bedside primary Registered Nurse (RN).
" · Discuss the patient's suitability for the Discharge Waiting Area and any specific needs they may have.

" · Ensure any barriers are identified and addressed
4. Educate Patient and Family
" · Ensure the patient and their family understand the purpose and benefits of the Discharge Waiting Area.
" · Provide a flier with detailed information about the Waiting Area and what to expect.
" · Address any questions or concerns they may have.
5. Identify Barriers to Timely Transfer
" · Assess and identify any potential barriers that could delay moving the patient to the Discharge Waiting Area.
" · Work proactively to solve these issues, ensuring a timely transfer.
6. Collaborate with Physician and Primary Nurse
" · Work closely with the physician and the primary nurse to obtain the discharge order promptly for those patients that have been identified as good candidates for the discharge Waiting Area.
" · Ensure all necessary documentation and discharge instructions are completed.
7. Assist on the Unit if able
" · Go to the unit to assist with the necessary preparations:
o o Assist with the After Visit Summary (AVS)
o o Help with removing any lines or IVs
o o Assist the patient in getting dressed
o o Assist with any teaching required
o o Pick up any required medications
" · Modified Staff follow job related restrictions
8. Coordinate Meals
" · Ensure any meals needed by the patient are prepared in a to-go box and on a disposable tray.
" · Facilitate the delivery of these meals to the Discharge Lounge.
" · Additional snacks/water are available via FANS who will stock the area on a routine basis. If supplies are low, please contact:

- Zion Senior -
9. Transport and Settle Patient
- Help move the patient to the Discharge Waiting Area.
- The front desk volunteers and or ambassadors can also help transport patients to the Waiting Area

· Ensure the patient is comfortable and settled in the Waiting Area environment.
· Ensure patients are signed in and out on the log kept behind the welcome desk
10. Document in KPHC using regional Smart phrase:
· This Smart Phrase is designed to streamline communication, enhance patient experience and ensure a smooth transition during discharge.
· Must use regional smart phrase ( DO NOT create a copy)
Document:
- if Arrival time in DC lounge
- if AVS was provided by the unit
- Time of DC lounge departure
+++ Use Military time+++
- If another staff member or volunteer supervises the patient in the lounge (and they are not the designated Discharge Nurse) ? Document - If another staff member escorts the patient out of the DC lounge to their vehicle ? Document their name and role.
11. Communicate with Family
- Engage with the patient's family as needed to keep them informed of the process.
- Address any concerns and provide support to ensure patient and family satisfaction.
- Ensure pick up time and location/plan are identified
Required Qualifications
PART TIME 20 GWW
Purpose:
· To provide a designated space for discharged patients waiting for transportation home .
· The DC waiting area aims to provide a comfortable space for our patients and improve overall hospital efficiency, improve throughput, reduce ED boarding, and support early discharge planning.
- Must be willing to float between the 3 San Diego facilities (SDMC/Zion/San Marcos)
- Must be willing to float between units within scope based on facility needs (may work in lower acuity if that is where the facilities need is)
- Scheduling is 2 weeks in advance (longer schedules cannot be requested)
- unit/facility may vary by schedule (Traveler may be prescheduled in lower acuity units)
- Potential to float between units/facilities mid shift
